REFLECTION ASSIGNMENT
Background
Over many years the organisation and delivery of nursing care has been forged by differing models of care as a result of economic and social changes (Tiedman & Lookinland 2004). Recently, the Garling Report (2008) and the NSW Models Of Care program (2006) triggered several model-of-care projects, including Take the Lead, WoW, and Essentials of Care. These represent professional nursings initiatives in devising improved systems of nursing care in response to recent developments in demand on health services by managerial developments, consumer needs, and the economics of health-care.
Duffield et al (2010) have shown, however, that nursing unit managers, faced with decisions regarding the organisation of care in their particular work setting, tend to base their decisions on pragmatic factors, such as skill-mix available, rather than any pre-conceived model of care.
Chiarella & McInnes (2010) point out that new styles of managerial control of large acute care hospitals have reduced the effective managerial scope of nurses, with consequences for the kind of care nurses can provide to patients.
What emerges is a situation of tension between perspectives shaped by nursings professional insight into optimal care for patients (reflected in the models of care), and managerial and resource factors which are real but not necessarily responsive to patient needs.
The Assignment
The Reflection Assignment asks you to draw on things you have engaged with in 92014 to develop an informed reflection on how consumer needs, skill mix issues, and demands on nursing decision-makers might affect the implementation of professionally devised models of care.
The task is this:
Choose one of the models of care (MOC) that you have been introduced to this semester: information about these can be found at the NaMO website (Dept of Health).
Using the reading provided, and the website information, consider the implications for your chosen MOC of the following things:
o The importance of a professional commitment by nurses to develop models of care which express nursing insight into optimal care for patients
o The actual and potential consequences for your chosen MOC of:
? Changes in the management arrangements of acute care hospitals
? The increasingly pragmatic styles of decision-making forced on unit managers faced with resource shortage problems
o The need to ensure that models of care reflect appropriate legal and ethical standards of care.
2

Prepare a written reflection on your understanding of these implications for the MOC, and
Close your reflection by addressing the following question:
o Given your evaluation of the impact of the issues listed on your chosen MOC, would implementation of this MOC allow proper expression of the ANMAC Nursing Competencies, or would it compromise these Competencies?

Set out your paper in four Sections. The topics for each Section are set out below.
Please do not write out the topic questions in your final submission: you need only indicate 1., 2., and so on, and then proceed to write your response.
1. The chosen Model of Care is

2. What is the main focus or purpose of this Model? Briefly indicate what elements are essential to it.
a. EXPLANATION: All the Models of Care are, broadly, concerned with patient care. However, each Model has a distinctive emphasis or focus. The distinctive focus and the aspects of the Model essential to it are what you must attend to here.

3. Discuss the prospects of effective implementation of the Model in light of the various constraints (clinical, legal, and ethical) described and reviewed in the available readings (required and other).
a. EXPLANATION: Here you need to use the required readings (see below).

4. Given your responses in 1-3, would the Competencies relevant to RNs be allowed proper expression with the implementation of the Model, or would they be compromised in some way? Give clear reasons.

REFLECTION ASSIGNMENT – How to understand the Grading Rubric
The Reflection Assignment Grading Rubric comprises three categories of evaluation. Below is an explanation of these categories, and how they apply to the Assignment.
Refer to the Assignment Information document while reading this explanation.
Category 1. Professional Dimension of Nursing
o The importance of a professional commitment by nurses to develop models of care which express nursing insight into optimal care for patients (Assignment Information)

What this Category asks: What professional attributes are needed, other than clinical competence, for the ability to develop nursing-based models of care?
Category 2. Professional Insight (PI) & Learning Experience (LE)
o The actual and potential consequences for your chosen MOC of:
? Changes in the management arrangements of acute care hospitals
? The increasingly pragmatic styles of decision-making forced on unit managers faced with resource shortage problems
o The need to ensure that models of care reflect appropriate legal and ethical standards of care.
Close your reflection by addressing the following question:
o Given your evaluation of the impact of the issues listed on your chosen MOC, would implementation of this MOC allow proper expression of the ANMAC Nursing Competencies, or would it compromise these Competencies? (Assignment Information)

What this Category asks:
(i) Professional Insight: using the required reading, are you able to identify, describe, and explain the likely implications for your chosen MOC of the managerial arrangements in acute-care hospitals, the necessity for pragmatic decision-making by NUMs, and the recommended legal/ethical standards to be met by any MOC?
(ii) Learning Experience: Do your responses in this section of the assignment show evidence of independent learning and development of understanding of the professional dimension of nursing? Are you able to convey a reflective evaluation of the implications written about beyond simply registering these implications?

Category 3. General Literacy
What this Category asks:
Have you used the core/required readings?
Have these been used in ways which contribute to development of your response, and not mere references?
Is the writing clear and coherent?
 Is the work organised well?
